For many seniors, there is a gap in medical care following a hospital stay for injury, illness or surgery. Often they are well enough to be released from the hospital, but not well enough to immediately return home. Farmington Presbyterian Manor bridges this gap with its Post-Acute To Home (PATH®) program for short-term rehabilitation in Farmington, Missouri, and the surrounding area.

PATH provides a place for seniors to transition from hospital to home and offers a variety of specialized services to help individuals regain functionality, and in many cases, return to their previous lifestyles.

With a doctor's referral and a qualifying hospital stay, PATH is open to any senior who qualifies for a post-hospital Medicare stay for recovery and rehabilitation.

Recovery and rehabilitation

PATH patients partner with a dedicated intake coordinator who works with physicians and family members throughout the admissions process. Next, a team of physicians, registered nurses, therapists and social workers tailors individual recovery programs to meet personal and medical needs. The team holds regular conferences with family members to ensure loved ones are kept informed of goals and progress. The referring physician also receives reports. The PATH program offers extended admission hours to meet the patient’s needs.

One notable aspect of the PATH program is its commitment to reduce hospital re-admissions. Our trained professionals complete thorough pre-admission assessments, help individuals reach discharge goals and follow up on progress after they have returned to their homes.
